How did it come about -- like, did you know it was happening before it got published? (I know nothing about podfic processes.)

Yeah!

I didn’t know this time, although I sometimes get asked first. My policy on works inspired by mine doesn’t require my permission. On AO3 when you submit a work you can specify that it’s inspired by another work. Then AO3 asks the creator of the original work whether they want to officially link them.
